Care Team Connect and Medical Advantage Group Power Care Across Michigan

EAST LANSING -- East Lansing-based Medical Advantage Group, a consulting and management services company providing support to physicians and physician practices, and its subsidiary, MAG Care Management, have selected Evanston, Ill.-based Care Team Connect to power a Web-based care navigation platform for the 140,000 patients served in 70 participating physician practices across Michigan.

MCM is one of the largest participants in the Michigan Primary Care Transformation Project. MiPCT is the largest of eight Multi-Payer Patient-Centered Medical Home Pilots in the U.S., with similar Primary Care Transformation projects also underway in Maine, Minnesota, New York, North Carolina, Pennsylvania, Rhode Island and Vermont.

Care Team Connect's secure, Web-based platform connects hospitals, community providers, family members and patients so they can work in partnership across the care continuum to improve patient outcomes while significantly reducing the overall cost of care.

MAG is a consulting and management services company providing expertise for a variety of physician organizations on the design and implementation of clinical integration strategies to maximize quality and efficiency outcomes. MAG has expanded its services through MCM to help health care providers manage the expanding demands of information and technology and the evolution of health service financing mechanisms.
