Care Team Connect and Medical Advantage Group Power Care Across Michigan
EAST LANSING — East Lansing-based Medical Advantage Group, a consulting and management services company providing support to physicians and physician practices, and its subsidiary, MAG Care Management, have selected Evanston, Ill.-based Care Team Connect to power a Web-based care navigation platform for the 140,000 patients served in 70 participating physician practices across Michigan.
MCM is one of the largest participants in the Michigan Primary Care Transformation Project. MiPCT is the largest of eight Multi-Payer Patient-Centered Medical Home Pilots in the U.S., with similar Primary Care Transformation projects also underway in Maine, Minnesota, New York, North Carolina, Pennsylvania, Rhode Island and Vermont.
Care Team Connect’s secure, Web-based platform connects hospitals, community providers, family members and patients so they can work in partnership across the care continuum to improve patient outcomes while significantly reducing the overall cost of care.
MAG is a consulting and management services company providing expertise for a variety of physician organizations on the design and implementation of clinical integration strategies to maximize quality and efficiency outcomes. MAG has expanded its services through MCM to help health care providers manage the expanding demands of information and technology and the evolution of health service financing mechanisms.
“MCM selected Care Team Connect as its technology partner for the MiPCT demo because we were impressed by their ability to support all of our functional requirements, including real-time care management and G code billing functionality, within the same easy-to-use web-based application,” said Donna Mimikos, MAG’s PO Consultant. “We are confident in Care Team Connect’s ability to scale with us to accurately support MCM’s more than 140,000 patients across Michigan.”
MCM Care Managers will enjoy the benefit of consistent protocol delivery while also supporting individual patient needs and engaging community resources across the 70 communities served by MCM supported physician practices.
“MAG is setting out to positively impact the lives of 140,000 patients, and we are honored that they have selected the Care Team Connect platform to help them meet their goals efficiently,” said Ben Albert, founder and CEO of Care Team Connect. “MCM and MiPCT’s focus is to help participants efficiently manage care, power and enhance care coordination and create linkages to community resources, and we are fortunate to play a role in this great program.”
